    Job Description:
    The RN CM is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. The RN CM will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
    Responsibilities:
    • Coordinates the patient care and identified patient needs with other services and departments throughout the hospital. Identifies patients who are at risk for adverse outcomes during the transition from one level of care/setting to another.
    • Gather information toward the care in relation to discharge needs and follow up care in accordance with the Utilization Management Plan.
    • Makes appropriate and timely referrals as patient need identified.
    • Demonstrates a current knowledge base in relation to diagnostic and therapeutic procedures required by patient or ordered by physician that is consistent with scientific principles, hospital and department policies, patient age and or developmental needs.
    • Demonstrates working knowledge of Interqual criteria as it relates to the patients record.
    • Collaborates with other health care team members, patient/family/SO, in developing the discharge plan.
    • Ensures continuity of care through precise and consistent sharing of patient information with other departments and staff on internal and external patient transfers.
    • Serves as an advocate for patients/family/SO regarding decisions affecting the Plan of Care.
    • Adheres to all regulatory and legal requirements in the performance of job duties. Example: JCAHO, OSHA, Federal and State regulations governing patient care and the practice of nursing.
    • Other duties as assigned
      • RN with current state licensure; BSN preferred
      • Three years' clinical, hospital nursing experience required with preference given to those with 2 years of case management experience or 2 years of critical care experience
      • Certification in case management or utilization review preferred
      • InterQual experience preferred
