Job Description Job Description Speech-Language Pathologist (SLP) – Skilled Nursing Facility | 2962524 Location: Titusville, FL A Skilled Nursing Facility is seeking a licensed Speech-Language Pathologist (SLP) to provide therapy services to short-term rehabilitation and long-term care residents. This is a full-time, permanent role requiring prior experience in a Skilled Nursing or post-acute setting. The SLP will evaluate, treat, and support residents with speech, cognitive, and swallowing disorders while ensuring regulatory-compliant documentation and interdisciplinary collaboration. Key Responsibilities Evaluate residents and develop individualized speech therapy treatment plans Provide therapy for dysphagia, communication disorders, and cognitive impairments Perform bedside swallowing evaluations and implement appropriate interventions Monitor resident progress and adjust treatment plans as needed Maintain accurate and compliant documentation in accordance with Medicare guidelines Participate in interdisciplinary care plan meetings and discharge planning Collaborate with nursing, rehab, and medical teams to support resident outcomes Required Qualifications Active Speech-Language Pathologist (SLP) license in the applicable state Certificate of Clinical Competence (CCC-SLP) preferred or required (depending on facility) Minimum 1 year of experience in a Skilled Nursing Facility or post-acute rehab setting required Strong understanding of skilled documentation and Medicare Part A / Part B guidelines Experience with dysphagia management and cognitive therapy interventions Ability to manage a productive caseload in a SNF environment Compensation & Structure Full-time, permanent position Competitive compensation based on experience Stable caseload within a Skilled Nursing setting Collaborative interdisciplinary rehab team
